First Visit Forms

Please prepare for your first appointment by completing our new patient information forms. Fill out the forms online. (We recommend you complete before your appointment). You can also update and change information at our NEW PATIENT KIOSK!  Please also create your username and password to access appointment times and update any contact information anytime.

Your First Visit

Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, payment policies and more. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Please be aware that your first appointment will include a detailed oral examination, and the necessary X-rays will be taken to make a proper diagnosis. Even if X-rays have been taken before in another office, and can be transferred to us, our protocol requires us to take our own X-rays for best readability and diagnostic quality. We will take these X-rays at no cost to the patient if there are insurance limitations. The radiation exposure of digital X-rays is minimal, and can be discussed in detail by the doctor if required.

We use the Soprolife, a special fluorescent intraoral camera that detects cavites which are not visible on X-rays. We also use the Velscope for early oral cancer detection. If we see excessive wear or cracked teeth, or there is history of TMJ problems, headaches and migraines, or history of gum disease, we will use the T-Scan to check abnormal forces on teeth and musculature.  We use a chairside test to detect bacteria that cause periodontal disease called the BANA test. We offer all diagnostic tests for $59 as they are not usually covered by insurance but are so important in early diagnosis and treatment.

All treatment options will be discussed in detail along with your insurance benefit, out of pocket expenses and financing options. After the exam treatment planning and agreement of payment options is completed, our scheduling coordinator will make you your first appointment which will usually be for the type of cleaning you were diagnosed with. We understand that our patients have a hard time making multiple appointments and want same day cleanings. The type of cleaning needs to be diagnosed,by the dentist first and the appointment individualized to each patient's needs, which makes it impossible for us to guess the length of appointment a patient needs with our hygienist. We do handle all emergencies on the same day and do full mouth or quadrant dentistry, thus making your subsequent visits minimal.

Payments for the day's appointment is collected and the next appointment made by the front office before you are taken back by the assistant.

Regular Checkups

We encourage regular checkups for individuals age four and up. Regular checkups are important to:

  • Check for problems that you might not see or feel
  • Find early signs of decay
  • Treat oral health problems early, while they are still manageable

Regular checkups include a thorough cleaning, polishing, and an oral exam by the doctor. During the oral exam, your doctor will check the health of your mouth, teeth, gums, cheeks, and tongue. Your doctor will also check your existing fillings and restorations because constant pressure from chewing, grinding, or clenching can cause them to wear away, chip, or crack.

Regular checkup appointments typically take about 45 minutes. Patients are seen by appointment only. We make every effort to be on time for our patients, and we ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately.
